I hearby authorize the staff of the Cameron Bairstow Basketball Camp 2015 to act for me and on my behalf according their best judgement in an emergency requiring medical attention.  I hereby waive any and all rights and release the Cameron Bairstow Basketball Camp 2015 from any and all liability for any injury or illness sustained by the above named participant while at the Cameron Bairstow Basketball Camp 2015 or the Lobo All-Star Game.  I have no knowledge of any physical impairment that would be affected by my child’s participation in the basketball camp or attendance at the Lobo All Star Game sponsored by the Cameron Bairstow Basketball Camp 2015.
